Monaghan Age Friendly Programme and Monaghan Integrated Development are delighted to officially launch the Older Persons Information Booklet. The booklet provides information on core services relevant to older people across the county. This booklet aims to support older people, their families and carers to have easy access to the contact details they need to access information.<\/p>\n

Age Friendly Programme<\/h3>\n

Monaghan has been an Age Friendly County for over a decade. Monaghan County Council signed up to become an Age Friendly County in September 2011.<\/p>\n

An Age Friendly County is a place where more and more older people can stay living in their own homes and communities, lead healthy and active lives, get to where they want to go, and are valued contributors to the lives of their communities.<\/p>\n

The Age Friendly programme involves a multiagency, multi-sectoral approach to age-related planning and service provision.<\/p>\n

Seamus McDermott was officially confirmed as the Age Friendly Ambassador for Monaghan at the Launch of the Croke Park Age Friendly Stadium in 2022. His role involves networking and encouraging community members to participate in local activities that cater to older people\u2019s needs and interests.<\/p>\n

Monaghan Older People\u2019s Council (OPC)<\/h3>\n

Monaghan OPC is a representative group of older people in the county. The Council identify priority areas of need, raise issues of importance and inform and influence the decision making process of the Monaghan Age Friendly Programme initiatives. Representatives of the OPC participate on the Monaghan Age Friendly Alliance committee.<\/p>\n

Monaghan Age Friendly Alliance<\/h3>\n

This is a high level, overarching strategic partnership that brings together representatives of older people in the county with the public, as well as voluntary, community, academic and private sectors. The Alliance oversees the realisation of the goals and actions of the Monaghan Age Friendly Strategy.<\/p>\n

Healthy Age Friendly Homes Programme\"Age<\/a><\/h3>\n

The Healthy Age Friendly Homes Programme aims to support older people to live in their own home with dignity and independence, for as long as possible. The programme\u2019s ambition is to enable everyone to live longer healthier lives by keeping care close to home and expanding the range of health and social care services in the community.\u00a0 It is about providing a range of alternatives for older people who may otherwise transition into long term residential care prematurely.<\/p>\n

To do this, we have appointed a Local Healthy Age Friendly Homes Coordinator in all local authorities.\u00a0 Each Local Coordinator will case manage a support package for each person referred, linking the supports from the variety of public, voluntary, private and community agencies in the area. We facilitate by providing supports on :\u00a0 Housing, Health, Finance, Community, Technology , Climate and Energy.<\/p>\n

The purpose of this referral is to identify an older person who could benefit from some extra help and support in order to continue living in their own home.\u00a0 The Local Coordinator will carry out a free <\/strong>home-based assessment and, in partnership with the older person, will identify and design a range of supports to enable the older person to continue living independently.\u00a0 This will not affect any current home help, medical card, pension, or other services being received.<\/p>\n
